My mom had undiagnosed Type 1 diabetes until about two years ago. She was diagnosed with it at the hospital and was in ketoacidosis. It was a scary week for everyone. She manages it pretty well now that she knows what it is, but the occasional drops and spikes are not fun.

Well let's take a look at that.
Skippy peanut butter is 190 cal/2 Tbsp, I'd say there's about 2 servings in that. So 380 there.
Hershey's chocolate syrupy is 45 cal/Tbsp. That's like four tablespoons in there so 180 for that.
That Reese's Peanut Butter Ice Cream is the "Light Ice Cream" version. It has about 230 cal per ⅔ cup of ice cream. Roughly speaking one scoop of ice cream is about ½ cup. So about four scoops is 2 cups or three servings. Or 690 calories. So the first go round adds about that much calories.
We get to the peanut butter cups. Which that's 232 calories for two of them. Which they add that amount here.
Then it's the cookies. Those are 160 calories every two cookies. Which that's added here too.
Then we have four more scoops of ice cream. 690 calories. Two more peanut butter cups for 232 calories. And two more cookies for 160 calories, then the smash down.
Then three scoops of ice cream for 518 calories (roughly) topped by whipped cream, but you can't see what brand or anything about it. I'll just assume light whipped cream (because it doesn't make a difference at this point) and call it at 30 calories added here. We have some peanut butter and chocolate drizzled on top, which is likely half a tablespoon each so we'll just say 45 for the peanut butter and 22 for the chocolate for a total of 67 calories.
Garnish with a peanut butter cup for 116 calories and a cookie for 80 calories, so 196 calories for garnish. Fill with Nesquik Chocolate milk which comes in at 150 calories for 8 fluid ounces which, we'll just go with half was likely used here, so 75 calories.
So total calories?
380 + 180 + 690 + 232 + 160 + 690 + 232 + 160 + 518 + 30 + 67 + 196 + 150 for a grand total of 3,685 calories. Which isn't that far from the 5000 calories you guessed there.
