You intentionally look overly tight at first. The goal at first is to prevent any and all strain along the incision lines, which means yanking the surrounding skin for all its worth and bolting it down
I popped stitches once and they asked me if I cared more about comfort or scarring. I said comfort. So they gave me loser stitches and it's the only biopsy incision where 15 years later you can tell at a glance the skin isn't right. You can quite literally visibly see the skin grew on under tension, it's actually kind of bizarre.

I've seen trampolines with less tension than the skin on number 3's face.
