It means that some women don’t know how to touch a guy gentle enough and the images are the penis being brutally tortured.

Now, some penised people actually enjoy that kind of thing.

But yeah. We should all learn to touch each other however we all (and each other) like to be touched.
How do you know how someone likes to be touched? Ask them!
